The North Carolina Health and Wellness Trust Fund Commission will award twenty (20) grants to reduce the disparities in the incidence, prevalence, and
mortality related to certain diseases in North Carolina, which are a result of race, ethnicity, and socio-economic status. The role of the Outcomes Evaluator
is to provide information to HWTFC in quarterly and annual reports regarding effectiveness, progress, implementation, and linkages of the grantees.
